In the event of chronic illness, preventive measures and diagnostic monitoring of the patient’s health condition are essential. Although patients suffering from chronic illnesses undergo clinical visits at regular intervals, there is no reliable data on how patients feel in the meantime, or how the disease is progressing. The absence of such data poses a particular challenge when it comes to efficiently selecting appropriate treatments, and cannot be remedied by individual patient feedback alone.

In order to reliably forecast disease progression, at DHPcare we rely on sensor-based, day-to-day patient monitoring and home-based patient-centered treatment approaches. Our method relies on AI-based biosignal analysis from clinical grade wearables that capture patients’ vital signs such as heart rate, blood pressure, breathing, etc. in real time and with clinical precision. Direct connectivity to the clinical environment means the data is immediately available.
